The story begins when Rose witnesses a patient take her own life while wearing an unsettling, fixed grin. Following this, Rose is haunted by terrifying, unexplainable visions of people smiling at her in a sinister way. As her mental state deteriorates, she discovers she has been targeted by a supernatural entity that "infects" its victims through trauma, leading them to suicide within days. The film concludes with a grim cycle as Rose is unable to escape the entity, eventually passing the curse on to her former partner, Joel, after he witnesses her death.
